Reputation drops when you attack Romulan ships or allies inside Romulan space, and building standing with the Federation or Klingonen pulls it down too.
Let it fall below Tolerated and entering Romulan space triggers a powerful hostile that attacks you as a trespasser, which is the practical reason to keep an eye on it.
There are five reputation locks, and once you pass one your Romulan standing can never fall below it again. The Fraktionsruf page has the full 35-tier ladder and every lock threshold.

- Commendations, which some missions require
Buy Commendations only while the mission that needs them is on your active mission list. They are easy to waste otherwise.

Working with more than one faction
You can build all three, but gaining reputation with one usually costs you standing with another. Most commanders specialise until they hit a lock, then switch, because the lock protects everything earned so far.
The other two core factions: Klingonisch, Föderation, and the Orion Syndicate sits outside the three.
